All information on form must be completed before it is submitted. The fraternity/sorority Recruitment/Intake Chair and President will receive your GPA and number of completed credit hours via email within forty-eight (48) business hours of submission.
THIS FORM MUST BE COMPLETED BY THE STUDENT WHO IS HAVING THEIR GRADES CHECKED!
